LifeSource Health Partners has many payment options. Our expectation is that services are paid for at the time they are rendered unless other arrangements are agreed to before your appointment.

Upon request, LifeSource Health Partners will provide a written explanation of estimated costs when we review your initial assessment and discuss treatment recommendations. A copy of our fee schedule is also available upon request.

Financial Policies

ChiroHealth and ChiroHealthUSAPlus:

are discount medical plans for the uninsured or underinsured. Since federal regulations do not allow doctors to charge patients who pay cash a different fee than insurance claims, we have entered into a contract with ChiroHealthUSA in order to keep our services affordable. By becoming a member of this organization, you and your entire family are eligible to enjoy deep discounts of up to 50% or more on most of our services. The membership fee is just $39.00 per year. MEDICAL Payment Insurance [MEDpay]: is a coverage included on most Colorado auto policy, unless rejected. This benefit allows the injured party to receive medical treatment as deemed necessary by your auto insurance company regardless of who is at fault.

In Colorado most policies provide for $5000 in medical payment benefits. However most insurance companies allow you to increase this amount of MEDpay at a reasonable cost. Consult your auto insurance carrier about available options. We at LifeSource Health Partners strongly recommend you increase benefit limits, because $5,000 doesn’t cover much medical care, especially if emergency services are needed.

MEDICAL LIENS: In most cases LifeSource Health Partners will accept a Medical Lien, when signed by you the patient, for services up to $5000 (higher if approved in advance). A Medical Lien allows us to treat you and wait for payment until settlement of the claim. If we do not receive payment within one year of your completion of treatment with LifeSource Health Partners interest will be charged at a rate of 1.5% per month. It is our policy to file all liens with the liability insurance carrier and the Colorado Secretary of State.

HEALTH INSURANCE: We are a non participating provider with health insurance companies and most health policies have a yearly deductibles and/or co-payments for both in-network and out of network providers. We will be happy to verify your benefits and complete all necessary forms for you to submit to the insurance company for reimbursement.

MEDICARE: At LifeSource Health Partners we are happy to serve Medicare beneficiaries. However there are many regulations and limitations to Medicare’s coverage of chiropractic treatments. They do not cover massage, acupuncture or routine chiropractic care. Medicare only covers spinal manipulation for treatment of an injury or certain health conditions. As a non participating provider, we are willing to complete and submit the forms needed for you to receive reimbursement.

HEALTH SAVINGS ACCOUNTS (HSA), FLEX and CAFETERIA PLANS: We are pleased to accept payments from HAS, Flex and Cafeteria Plans when applicable. We will provide you with an itemized statement upon request.

Neck

We are affiliated with Care Credit that may help with the costs of whatever treatment you select. This requires completion of an application and a credit check. CareCredit then determines if and how much credit they are willing to approve. This option can be very helpful in minimizing the total cost of treatment because it allows you to take advantage of our prepayment discount while still using their interest free option.

Ultimately, you the patient are responsible for the fees associated with all treatments received. Past due accounts will accrue interest of 1.5% per month or the maximum amount permitted by law. We reserve the right to pursue all legal remedies.

SUPPLEMENTS: Payments for supplements are due at time they or received or ordered. Returns are accepted only if returned within 30 days of purchase, unopened and have at least 9 months remaining on the expiration date. The refund will be given in the form of a credit toward services.
